fatal tenofovir-associateacd lactic acidosis: a case report

چکیده
case presentation a 52-year-old malay gentleman, with hepatitis c virus and hiv infection was admitted to the intensive care unit for severe lactic acidosis, with concurrent escherichia coli bacteremia with multiorgan dysfunction. the patient was started on highly active antiretroviral therapy, which included tenofovir, 5 weeks before presentation. antimicrobial therapy, continuous veno-venous hemofiltration, and other supportive treatments were instituted. however, the patient eventually succumbed to his illness. conclusions it is essential for clinicians to be able to recognize the signs and symptoms of lactic acidosis in nrtis treated hiv patients, as an early diagnosis is important to institute treatment. introduction the introduction of highly active antiretroviral therapy (haart), in 1996, has resulted in marked reductions in the rate of illness and death, due to hiv infection. the haart has transformed hiv infection into a manageable chronic disease. however, although many regimens lower plasma viral load, to below the limit of detection, in most patients, maintaining viral load suppression remains challenging, because of adverse effects and toxicity in the long term, which can lead to non-adherence, virologic failure and drug resistance. although rare, lactic acidosis often develops fatal complications, as reported in several human immunodeficiency virus infected patients treated with nucleoside reverse transcriptase inhibitors (nrtis). the purpose of this paper is to report a case of tenofovir induced lactic acidosis and review the literature.

متن کاملMetformin-Associated Lactic Acidosis: A Case Report
A 54-year-old woman with type 2 diabetes mellitus, hypertension, and peripheral vascular disease developed life-threatening lactic acidosis during treatment with metformin for type 2 diabetes. The woman received metformin at 1000 mg orally twice a day for type 2 diabetes. She presented to our emergency department with a 3-day history of severe watery diarrhea, nausea, and vomiting. Her grandson...
متن کاملCase report: lactic acidosis and rhabdomyolysis during telbivudine and tenofovir treatment for chronic hepatitis B.
BACKGROUND Current treatment options for chronic hepatitis B (CHB) are pegylated interferon alpha and nucleoside analogues (NAs). NAs have relatively fewer side effects than interferon alpha, and generally well tolerated. Previously 12.9% of patients on telbivudine treatment were reported to develop severe elevation of serum creatine phosphokinase (CPK) levels, but related clinical disease, lik...
متن کاملViekira Pak Induced Fatal Lactic Acidosis: A Case Report of an Unusual Side Effect
Viekira Pak is a new direct-acting antiviral agent that has an excellent efficacy in treating patients with chronic HCV. FDA released a safety warning that Viekira Pak can cause serious liver injury mostly in patients with underlying advanced liver disease. We report the first case of fatal lactic acidosis presenting 3 days after initiating therapy with Viekira Pak. Although it is very hard to ...
متن کاملFatal lactic acidosis in hepatitis B virus-associated decompensated cirrhosis treated with tenofovir
RATIONALE Recently tenofovir disoproxil fumarate (TDF) has been widely used as a first-line therapy for chronic hepatitis B (CHB) infection. Although TDF demonstrates successful viral suppression, the possibility of renal failure and lactic acidosis has been proposed with TDF administration, especially in human immunodeficiency virus co-infected patients. However, TDF induced lactic acidosis ha...
متن کاملLactic acidosis secondary to metformin overdose: a case report
INTRODUCTION Metformin is a commonly used treatment modality in type 2 diabetes mellitus, with a well documented side effect of lactic acidosis. In the intensive care setting lactate and pH levels are regularly used as a useful predictor of poor prognosis. In this article we highlight how high lactate levels are not an accurate predictor of mortality in deliberate metformin overdose. CASE PRE...
